Welcome to our Forms page. Here you'll find all the forms you will need to join the Association, register, transfer, lease, or do any reporting required. Fill in all the appropriate information and mail, email or fax it to:

In order to simplify our processes, the CSHA is making changes to the way we send and receive information pertaining  to our membership, registrations and other forms.  Many of the forms below are becoming “fillable” and can be opened and saved using Adobe Reader. Just download the form to your computer, enter and save your information, and email the completed application to the Secretary at cshasec@pris.ca. Please also note the following:

  • Members: Where a signature is required, please type in the appropriate name.  In order to verify the signature, the form must be sent from the email address that is on file with the Association. 

  • Non – Members are required to print the completed form, sign and mail, scan and email, or fax the document.

  • Documents such as the Certificate of ID, CSHA Veterinary Stallion Inspection and Shire Cross Description must be printed, completed and signed as required, and mailed, scanned and emailed, or faxed.

We have set up a PayPal account for the CSHA.  For many this is a convenient way to send fees to the Association and to speed processing.  As always, forms cannot be processed until the fees have been received.  If you are not comfortable making payment through PayPal on the website, you can send the forms to the Secretary and request that a PayPal invoice be emailed directly to you.

Of course, if you are uncomfortable with this technology, you are welcome to complete your forms, include your cheque or money order made out to the Canadian Shire Horse Association and mail to the CSHA Secretary at the address above.

Registration

The term “Breeder” applies to the owner or lessee of the named Dam at the time of foaling.  The Breeder is responsible for CSHA registration or Colt Notification of the Foal.

All horses (except geldings) must be DNA tested: Request for DNA Parentage Verification. This rule applies to all horses entering the Mare (Filly) and Stallion Registers. The Rule will not apply to horses entering the non-breeding register.

General
It is not necessary to be a Member of the CSHA in order to register and have an entry in the Stud Book. However, fully paid up Members of the Association enjoy preferential fees.

Every animal must have a registered prefix. If new breeders do not already have a prefix, please contact the CSHA Secretary to ensure your chosen Prefix has not been previously registered.   Complete the Application for Registration of Prefix and submit the onetime fee. Duplication of names is not permitted and numerical symbols are not allowed.

All details of the Certificate of ID must be fully completed, and the written description and the sketch must agree. The Certificate of Id must comply with the FEI Identification of Horses as identified on the Canadian Inspection Agency website.

To register a Stallion - The CSHA Veterinarian Stallion Inspection form must be printed, completed and signed by a Veterinarian. The completed form may be scanned and emailed to the Registrar by the Veterinarian’s office.  All Colts must be inspected by a qualified Veterinarian to ensure they comply with the “Breed Standard”, prior to being issued a Stallion Registration.  Please provide a copy of the Breed Standard and FEI Identification of Horses to your Veterinarian. 

A completed Stallion Service Report must be on file from the owner of the animal’s Sire.

The Canadian Shire Horse Association recognizes as purebred:  Any Shire that is fully registered in the Studbook of the Canadian Shire Horse Association (CSHA), The Shire Horse Society (SHS), the American Shire Horse Association (ASHA)or any progeny of these same Shires.

To Register a Purebred Shire – Female

  • The Application for Registration form must be completed by the Breeder (owner of the Dam at time of foaling).

  • The Certificate of Identification must be completed and signed by a Veterinarian. The completed form may be scanned and emailed to the Registrar by the Veterinarian’s Office.

  • Complete the Request for DNA Parentage Verification Kit.  This kit, with instructions for completion, will be emailed to you.  Include hair sample as directed and mail to Veterinary Genetics Laboratory.  The result will be emailed to the CSHA Secretary.

  •  A completed Stallion Service Report must be on file from the owner of the animal’s Sire.

  • The following fees must be submitted with the Application:

  Non-Members Members

Female animals Registered before Dec 31 of the foaling year

Female animals Registered between Jan 1 and Dec 31 of the yearling year   $100 $50

To Notify a Purebred Shire Colt

  • The Application for Registration form must be completed by the Breeder (owner of the Dam at time of foaling).

  • The Certificate of Identification must be completed.  Veterinarian signature is not required. The completed form may be scanned and emailed to the Registrar by the Veterinarian’s Office.

  • A completed Stallion Service Report must be on file from the owner of the animal’s Sire.

  • The following fees must be submitted with the Application:

To Register a Purebred Shire – Stallion

Registrations when also registered with SHS and ASHA

To Register a Purebred Shire – female - that is fully registered with the Shire Horse Society:

  • The Application for Registration form must be completed in full.

  • Enclose the original SHS Registration papers with the stamped Transfer Certificate that certifies the applicant is the registered owner.

  • Enclose a copy of the DNA Certificate with Parentage Verification.

  • The fee of $25 for Members or $50 for Non-Members must be enclosed with the Registration, or use the PayPal option below at the end of this section.

  • The Application for Registration form must be completed in full.

  • Stallions with SHS registration papers, who have been previously examined by a Veterinarian and approved will not require a second Veterinary inspection.

  • Complete the Stallion Service Report – List all Mares covered by the Stallion including Registered Shire Mares, Recorded Shire-Cross Mares and Grade Mares.
