Membership Form

This form is for those who wish to become an Annual or Life Member.

Please note that you must own a Shire horse that is registered in the Stud Book of the Canadian Shire Horse Association.

Annual Membership = $50.00

Life Membership = $500.00

Associate Membership

Application for Associate Membership.

Applicant does not own a Shire horse that is registered in the Stud Book of the Canadian Shire Horse Association.

Annual Membership = $30.00

Life Membership = $500.00

Transfer of Ownership - Notified Colts

This form must be accompanied by an original Certificate of Colt Notification, and a Certificate of Identification.

Transfer Fee: $20.00

Stallion Service Report

Report for stallion owners, for the breeding season. This form must be submitted by December 31st of the current year.

*NEW Registration of Prefix- The official form for registering a prefix for naming your horse(s), according to the CSHA constitution. NEW*

Registration Procedures- Procedures for registering your Shire with the CSHA.

Registration Form 1 and Registration Form 2. Please note that both forms must be included with an Identification form.

Identification Form- Must be included with the Registration forms, and it MUST be filled out by a veterinarian.

Veterinary Exam Form '1' - For your vet to fill in, this must be included with vet form '2'.

Veterinary Form '2' - Part two for your vet to fill in. Must be accompanied by vet form '1'.

Shire Standards - Print this form for your veterinarian to follow when checking your horse. These are the standards to be followed, as set forth by the CSHA.

Shire-Cross Registry Forms - This package contains four (4) forms for Shire-Cross registration, transfer, recordation and description.

Procedures Form - Recordation procedures for Shire-Cross

Transfer of Ownership - Shire-Cross Transfer. Note that transfer fee is $20.00

Certificate of Recordation - Includes Certificate of Breeding section, and Owner's Certificate.
